One practice, two ways in

Start at the door that fits what you need.

One practice, two ways in. Therapy plus medication care lives at psych.us. Medication management only lives at psychmedcare.com. Locations, insurance, fees, and booking for both live here.

Therapy and medication care

Psychotherapy, or therapy and prescribing together, from one clinician who hears the whole story. This is the full practice, including OCD and trauma care.

Therapy and medication care: the full practice at psych.us

Medication management only

A dedicated prescribing practice: careful evaluation, precise medication work, GeneSight testing and labs when they help, and Follow-up Appointments with real time in them.

Medication management only: PsychMed Care

How the practice runs

A small practice, run on purpose.

One clinician

Every visit is with Karel Winner, DNP, PMHNP-BC: a doctorate from Vanderbilt University, psychiatric training at the University of Tennessee, and two decades in medicine. No rotating roster, no handoffs between strangers. The person you meet at the Comprehensive Initial Appointment is the person who knows your chart at every visit after.

Telehealth first

Visits happen by secure video across 34 states and Washington DC, so care continues if you move, travel, or change jobs. Seven offices anchor the practice on the ground. One note for patients prescribed controlled substances: federal and DEA rules can require an in-person visit, typically once a year, and when that applies we arrange it in the state where you are, planned well ahead of time.

Verified before you start

Insurance is checked before the first appointment, and you hear your exact copay before you commit to anything. Fees are published, not negotiated. And because health information deserves better than a web form, booking and intake run through IntakeQ, a system built for medical privacy.

Your first month

How getting started actually goes.

Before the visit

You book on IntakeQ, complete the intake forms, and send your insurance card. We run the eligibility check right away and tell you your exact copay. If anything about coverage is unclear, you hear it from us before the appointment, while there is still time to decide.

The Comprehensive Initial Appointment hour

Sixty minutes with your clinician: your history, the treatments you have already tried, what helped, what did not, and what you want care to do for you. By the end of the hour there is a plan, and it is one you helped make.

Settling in

Follow-up Appointments are scheduled at a rhythm that matches the plan, usually every few weeks at first. Between visits, the patient portal and Spruce carry your messages, and your records, labs and medication history stay in one chart that your clinician actually reads.

Your clinician

One name on the door.

More about the practice.

Ready when you are.
